Telehealth ROCKS Impact Reports

The Telehealth ROCKS program works with network partners to place School-Based Community Health Workers (SB-CHWs) in school districts to address social determinants of health needs for students and families. Interactions, needs and assessment outcomes are tracked and compiled into impact reports.


Telehealth ROCKS School-Based Community Health workers assess social determinants of health using the Arizona Self-Sufficiency Matrix, a validated tool to assess an individual or family’s progress over time across 18 domains.
